Photo Release Waiver

This Liability Waiver and Release ("Agreement") is entered into between the undersigned client ("Client") and  Transitional Roots LLC, its owners, employees, agents, and contractors ("Salon").

By signing this form, the Client acknowledges and agrees to the following terms and conditions:

1. Assumption of Risk

The Client acknowledges that certain activities involved in hair salon services, including but not limited to hair cutting, coloring, styling, chemical treatments, and scalp massages, carry inherent risks of injury, including but not limited to burns, allergic reactions, cuts, or other conditions that may result in discomfort or injury.

2. Release of Liability

To the fullest extent permitted by law, the Client releases, discharges, and holds harmless the Salon, its owners, employees, agents, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from or in connection with any injury, loss, or damage resulting from services provided at the Salon, including but not limited to personal injury, allergic reactions, burns, or any other physical condition arising from the services rendered.

3. Health Information Disclosure

The Client agrees to disclose any relevant medical conditions, allergies, or sensitivities to the Salon prior to receiving any services. The Client acknowledges that failing to disclose such information may increase the risk of adverse reactions or injury.

4. Waiver of Claims

The Client agrees to waive any and all claims for injury or damage, whether caused by negligence or otherwise, against the Salon and its employees or agents. This waiver includes but is not limited to any claims for medical costs, lost wages, or emotional distress arising out of the salon services.

5. Indemnification

The Client agrees to indemnify and hold harmless the Salon from any claims, demands, or causes of action, including reasonable attorneys' fees, arising out of or in connection with the Client’s use of the Salon’s services.

6. Acknowledgement of Understanding

The Client confirms that they have read this waiver thoroughly, understand its terms, and voluntarily agree to its contents. The Client acknowledges that they have had the opportunity to ask questions regarding the waiver and that they are signing this Agreement of their own free will.

7. Severability

If any provision of this Agreement is found to be invalid or unenforceable by a court of law, the remaining provisions shall continue in full force and effect.

8. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of Florida.

If a client is under 18, a parent or legal guardian may sign on their behalf.

This authorization is signed on September 11, 2025, and shall remain valid until revoked in writing.
